Next Level Golf @ TGA+ Agreement

ACKNOWLEDGMENT OF RISK AND RELEASE OF LIABILITY Member/Non-Member/Players and their Guests understands that there is an inherent risk associated with the use of the Next Level Golf @ TGA+ facilities, equipment and programs, that may result in an injury to the Member/Non-Member/Player and their Guests. Therefore, the Member/Non Member/Player and their Guests assumes full responsibility for any injury or damages to the Member/Non Member/Player or Member/Non Member/Player’s guests in, on or about Next Level Golf @ TGA+ premise and does hereby and forever release and discharge The Next Level Golf @ TGA+ Management, Inc., The Next Level Golf @ TGA+, its owners, employees, and agents from any and all claims, demands, damages, rights or causes of action, present and future, whether the same be known or unknown, anticipated or unanticipated, resulting from or arising out of the Member/Non Member and the Member/Non Member/Player’s guests use, or intended use, of the facilities, equipment and programs.

Senior, Family, Simulator/ Range Membership: I understand that this agreement is a month-to-month Membership paid each month by 1st calendar day of month or auto deduction of membership price based on the payment type registered at the time of account access, and I acknowledge and accept the terms of the Term Section in the Membership Agreement. Membership may be terminated at any time with thirty (30) days' notice by completing and sending the Cancellation Request Form in person or by certified or registered mail to the Next Level Golf @ TGA+
