Member, Parent Or Guardian General Undertaking

1. I agree to comply with the rules of the club as posted by the club at the club and on the website. Rules may be enforced by any member of the Executive, the club pros and court monitors.

2. I understand that my membership may be revoked or suspended for non-compliance of club rules.

4. I hereby release Gwendolen Park Tennis Club, its Executive, employees, volunteers, and members from any liability for loss, damage, or injury (including death), however caused, that may result from participation in club activities and declare that this release is binding upon my heirs, executors, administrators, and assignees.

Mandatory Release

On my behalf (and on behalf of my children), I hereby release, covenant not to sue, discharge, and hold harmless the Club, its employees, agents, and directors, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of the Club, its employees, agents, and directors, whether a COVID-19 infection occurs before, during, or after participation in any Club program or activity.

I hereby release Gwendolen Park Tennis Club, its Executive, employees, volunteers, and members from any liability for loss, damage, or injury (including death), however caused, that may result from participation in club activities and declare that this release is binding upon my heirs, executors, administrators, and assignees.

BY SIGNING THIS DOCUMENT, YOU WILL WAIVE CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE OR CLAIM COMPENSATION.I ACKNOWLEDGE THAT I HAVE READ AND UNDERSTOOD ALL OF THE TERMS OF THIS AGREEMENT AND THAT I AM VOLUNTARILY WAIVING SUBSTANTIAL LEGAL RIGHTS (ON MY BEHALF AND ON BEHALF OF MY HEIRS, EXECUTORS, ADMINISTRATORS AND NEXT-OF-KIN), INCLUDING THE RIGHT TO SUE THE CLUB AND THE RELEASES.

I CONFIRM THAT I HAVE EITHER SOUGHT AND RECEIVED INDEPENDENT LEGAL ADVICE OR I AM WAIVING MY RIGHT TO SEEK INDEPENDENT LEGAL ADVICE PRIOR TO SIGNING THIS AGREEMENT.

I agree to all the terms and conditions set forth in this Agreement.
